Nav-CARE is a free program offered by volunteers of the Teresa Dellar Palliative Care Residence designed to improve the quality of life of people living with serious chronic illness, and their caregivers. Designed for those who have quality of life concerns such as:
•Loneliness or social isolation
•Recent loss or multiple losses
•Mobility or sensory challenges
•Coping with transitions and multiple decisions
•Difficulty finding or accessing information or resources.
